The core idea might not be in the inner monologue absence, i suspect NPCs just can't attribute thoughts to their source: When a NPC gets a thought, the NPC assumes that the thought is being created by his own will and not from a process in the mind or from external influences. The enables programming to bypass "attribution check" that judges information sources - as the NPC believes all thoughts are coming from its own I-mind. Inner monologue however helps to put the distinction from conscious thoughts(coming from willful contemplation on a topic) and generated thoughts(coming from memory, external influences/senses, background processes). Without the inner monologue every thought is read with the same non-verbal mental recall circuit that lacks "color" of thought: voices add "color"/"temperament" which helps to distinguish between sources of thoughts.
Here is catch: Inner monologue doesn't guarantee that the person can distinguish the source of voices/thoughts, he can be assuming each voice is the same channel with different topic. Its a skill that has to be learned or developed. Discriminating mind can "entertain a thought without accepting it." because it knows that the thought isn't coming from its will.

Also, the label NPC is the most apt description of the phenomena: 1.Its neutral and non-political(except the implied "dehumanization") 2.Its readily understood due gaming being popular 3.It hints at true nature of humans 4.It doesn't introduce any complex ideas The only flaw here is black/white division of NPC/PC, since its likely a spectrum similar to Autism and to less extent Dyslexia. "High-functioning NPCs" may live through sub-conscious programming without expending much mental effort, like some meditators turning off "discriminating mind" to reach unfiltered "pure consciousness". In the coming months, the semi-conscious NPCs will absorb the meme and will start asking themselves questions: Am i a NPC? How does my mind actually works? What is the source of my thoughts? What directs my thinking? The psychological realization will be traumatic, i believe it may even cause suicides and extreme escapism(drugs/VR). The NPC sentiment is that "NPCs are programmed by the mainstream media", as if the media is able to send commands into their brain. That would mean some sort of deliberate hypnotic suggestion and requires constructing the media programs around hypnosis - something that would readily apparent in analysis.
A more plausible explanation is brain damage from an environmental factor: xenoestrogens,microplastics, stress and microwave radiation(cellphones/wifi) impairing specific cognitive circuits(lesions, cell death) so they can't discriminate own thoughts from processing of incoming/absorbed information.
