The law is usually justified by remembering that the healthcare costs you will require if in an accident without a seatbelt are very high and will need to be footed by the taxpayer in almost every circumstance.

Refusing medical treatment to somebody because they weren't wearing a seatbelt is not possible because 1) refusing to perform reasonable emergency life-saving help for any reason is essentially manslaughter and sets a bad precedent in regards to the value of life and 2) how would they tell on the spot? What if your seatbelt just failed?

Sure, they could perform the healthcare required on the spot and find out through forensics whether they were wearing a seatbelt, billing them if they weren't, but people simply can't pay those bills -- they can't give back the space and specialists who were required to clean up after them, possibly at the expense of less reckless patients, never mind the astronomical immediate monetary costs.

Wearing a seatbelt has been proven to make such a massive difference in an accident, more than doubling your chance of avoiding any injury, that refusing to wear one is a very questionable choice -- so reckless as to make one question your judgement and mental health. You're lucky that you only have to pay a fine for not wearing a seatbelt, displaying that level of reckless disregard should really be enough to get you removed from polite society permanently.
