Well, it could be exactly as the dead person says. No reason not to. On the other hand they're fucked up enough to kill themselves so any claim that they're acting rationally should require extraordinary evidence. If said person also had their dick and balls cut off previously (or wasn't quite that far down the road), that's more reason to think they've been untreated for quite a while longer.
